Registration and storage of data
When you are a client of mine, it is necessary for me to register and use certain personal data about you. In addition to what follows from i.a. the Record Keeping Order and the Health Act/Psychology Act, the purpose of the registration is to give you the best possible treatment.
The personal data I register originates from you, or – if your treatment has started through a referral from a business partner – also from them. I do not combine your data with information from other actors, e.g. Social Media.
I have a duty of confidentiality, and the information I register and use about you, I have a duty to store securely by appropriate technical and organizational measures. Information about your health conditions is only passed on to persons and actors outside the clinic with your consent.
After you have been in contact with me for the last time, I am obliged to keep your records for at least 5 years. If you choose to change psychologists, I can, with your consent, transfer your records to the new psychologist, who will then take over the duty to keep your records.
For accounting reasons, it may also be necessary for me to register and use certain information about you, e.g. to be able to complete the payment for your treatment via the Jobcentre in your municipality or an insurance company. This information is registered on the basis of Art. 6 pieces. 1, letter b and fi of the Personal Data Regulation. I only keep them for as long as is relevant and required for my settlement and bookkeeping.
Access to documents
You have the right to access the information I register and use about you at any time.
If you believe that I have registered incorrect information about you, you can ask me to correct the information. I am not allowed to delete information in your record, but if you think there is an error in the record, you can ask me to make an addition with your comments.
Complaints
If you wish to complain about my processing of your personal data, you can complain to the Danish Data Protection Authority. More information on this can be found at www.datatilsynet.dk .
